Ask for a copy of your personal data, a correction, its erasure, a restriction, or withdraw your consent — whether you are a patient, sent us a message on this website, or wrote to us on social media.

What happens next

You receive a reference number (and an e-mail confirmation link if you gave an address).
We check that the request comes from you — for example by a reply from the e-mail or phone we already hold, or an ID at the clinic.
We answer within 30 days. A copy of your data is sent as an encrypted file; the password is sent separately.
Medical records the law requires us to keep cannot be destroyed early; we restrict their use instead.
