## Abstract

Doctoral thesis supervised by Prof. Hamdi Abo El Fotouh Hamed and Assoc. Prof. Ahmed Emad Fayyad (Cairo University) and Dr. Mohamed Marwan Rahhal (Fayoum University). Thirty-six patients with thin mandibular ridges from the outpatient clinic of the Prosthodontics Department, Cairo University, first received conventional complete dentures and were then randomized into two groups of 18. The intervention group received two mini-implants (2.5 mm wide, 10 mm long) in the canine region; the comparator group received two conventional implants in the canine region after ridge splitting with discs and engine-driven expanders, placed with a two-stage protocol and three months of healing. Both groups received mandibular overdentures retained by ball abutments. Satisfaction (overall, stability, aesthetics, chewing efficiency) was recorded with a VAS questionnaire, and bone height changes were measured on standardized long-cone radiographs (Digora) at baseline, 6 and 12 months. Both treatments improved satisfaction, with higher satisfaction in the mini-implant group; both groups showed bone resorption, considerably greater in the conventional implant group. Conclusion: a mandibular overdenture retained by two mini-implants is a successful option with high patient satisfaction and less bone height change, and may be easier and less traumatic than ridge expansion in thin ridges.
